Grooming Client Agreement

Our goal at Groomingdale’s Salon and Paw-tique, LLC is to provide quality care for your pet to create the best possible experience. We understand that many dogs do not enjoy the grooming process. At drop off we see dogs that show signs of being nervous and reluctance, we strongly suggest that the owner does not stay with the their pet. Many times after their owner leaves they are more comfortable with us. During their time with us we want to be aware and respectful of their emotional state, it is the only way to establish trust. We don’t expect them to love everything we do, but we want them to know that we are not here to hurt them. Ending on a positive note every visit is what builds our relationship and lowers stress.

On average we like to have the dogs for about 4 hours. This ensures that your pet does not have to stand the entire time, great for seniors and puppies. For thick double coated breeds they don’t have to stand for hours of drying; especially nice for dogs that are afraid of dryers. For curly coats, that need to be straight and fluffy to ensure an even haircut,  they can rest before grooming is finished. If you would like your dog to be done start to finish there is a rush fee of $25.

We track the grooming instructions so we can provide consistency. If after any appointment the grooming is not to your expectations, please notify us within 48 hours and we can make necessary arrangements too correct the situation. If it is a medical issue we will need to have written notice from your vet that it was caused by grooming. Grooming can aggravate pre-existing issues, please let us know when scheduling and/or at drop off if you have concerns, have noticed changes in behavior, or minor injuries.

Please arrive on time for your appointment. If you are going to be more than 5 minutes late please call to notify us. If you are arriving more than 20 minutes late we may have to reschedule your appointment, this will be considered a cancellation. If you cancel with less than 24 hours notice on more than one occasion you will be required to prepay for future appointments. If you miss your prepaid appointments or give less than 24 hours notice your payment will be forfeited.

Payment for services rendered is required upon pickup; we accept cash, check, and all credit cards. Prices may increase without notice.  Example:  Matting, length of time between visits, etc.

We expect our grooming clients to be picked up by 5:30 P.M. unless it has been arranged prior to your appointment.  If you pick up after our last daycare dog, there will be an additional charge.

Feline Grooming Agreement

My pet is healthy and current on all vaccinations.  Any grooming which takes place on an elderly cat or a cat with existing health problems will be at the owner’s risk.  Grooming can also expose pre-existing health and/or skin issues for which Groomingdales cannot be held liable.  In the event of injury or a health concern, I permit Groomingdale’s to contact a vet and I also authorize any veterinary care required for my pet as necessary.  I agree I am responsible for any and all vet charges that may result from such care.

Shaving your cat will significantly change your cat’s appearance and the hair will be very close to the skin.  De-matting /detangling of matted hair may cause patchiness and /or hair loss.  I understand this and give permission for the matting to be removed.  Severe matting and or pelting will result in an additional charge based on the severity of the problem and the time required to correct it.   A cat-i-tude surcharge may also apply to those kitties requiring an extra helping hand.

Daycare/Boarding Agreement

Groomingdale’s Salon and Paw-tique, LLC (hereafter referred as GS) and the owner of the dog(s) to be admitted into daycare (hereafter referred to as Owner) hereby agree as follows

  •   Owner certifies that to the best of his/her knowledge, the dog’s vaccinations are current, and the dog is in good health, not having been recently exposed to any contagious canine illnesses prior to any future stays at GS daycare for a minimum. In admitting the dog to daycare at GS, Owner understands and agrees that GS, it’s officers; owners, employees and agents have relied upon Owner’s representations that the dog is in good health and currently vaccinated and Owner’s representations regarding the dog’s behavior history.
  • GS will exercise reasonable care of the dog during its stay. Dog Owner does acknowledge and he/she understands that their dog will co-mingle with other dogs. In consideration of these services, Owner expressly waives and relinquishes any claims against GS, its officers, owners, employees and agents, relating to services provided pursuant to the Owner’s dog, except those claims arising from willful misconduct by the GS, and further agrees to indemnify GS against all such claims.
  • The Owner is aware of and understand the techniques used in the dog training programs offered at GS and if the Owner chooses to have GS train his/her dog, consents to the use of such techniques on his/her dog and waives and relinquishes any claims against GS, its officers, owners, employees and agents relating to any training provided pursuant to this agreement, including damage or injury to the Owner’s dog or injury caused by the Owner’s dog, except those claims arising willful misconduct by GS and further agrees to indemnify GS against all such claims.
  • Owner further understands and agrees that in the event of an emergency, GS is authorized to seek emergency veterinary care as GS deems necessary and GS is hereby released from all liabilities related to the transportation, treatment and expense relating  there to. In the event of death of Owner’s dog while under the care of GS, Owner authorizes GS to have the body taken to a veterinarian for storage, at Owner’s expense, to await disposition instructions from Owner.
  • Owner agrees to pick up his/her dog by 6:45 pm and understands that Owner will be charged a fee of $10 for their dog(s) not picked up by 7:00 pm, as a boarding fee and GS, its officers, owners, employees and agents, will leave for night. If the Owner calls and wants GS, its officers, owners, employees or agents, to stay after hours there will be $1/minute charge to the Owner. If dogs are not picked up within 3 days, or if GS does not receive contact from Owner during that period, the dog(s) shall be considered abandoned and GS will place the dog(s) at Owner’s expense, as GS deems appropriate. Owner further agrees to release, hold harmless and indemnify GS for any claims relating to the transportation and kenneling of the dog(s) should Owner fail to pick up the dog for any reason prior to closing.
  • Owner further agrees that in the event that GS should observe/suspect that his/her dog may appear to be ill in any way, as determined by GS, its daycare staff, or Stacey Hess, GS will require a Veterinarian’s note verifying the health status of the dog before allowing the dog to return to the facility. No refund shall be issued for unused pre-paid daycare days. Owner shall be responsible for picking up the dog from daycare within a reasonable period of time on the same day, after being notified by GS that his/her dog needs to be removed from daycare. The dog will not be allowed to return to Daycare until a Veterinarian has provided written health clearance.
  • Owner agrees to the following conditions regarding pre-paid cards: Pre-paid daycare visits are non-refundable. Pre-paid daycare visits expire 180 days  from the date of purchase. Unused days will be forfeited and will not be refunded or honored after the expiration date.
  • Owner agrees to the following conditions regarding Easy Pay Monthly Membership: For discounted rates on daycare visits you can sign-up for automatic payment with a credit card on file, your card information is encrypted. Your credit card will be charged on the 5th of every month. You will choose the number of visits, half or full, you would like per month (4, 8, 12, 16), if you go over the amount of visits you purchased, those days will need to be paid individually at check out. Unused days will be forfeited and will not be refunded or carried over into the following month.  If you choose to go to a different size package or unsubscribe from Easy Pay Monthly Membership you will need to give GS a minimum of 5 days before the the billing date.
  • I give my permission for photos of my dog to be taken by Groomingdale’s Salon and Paw-tique, LLC staff for use in advertising and promotional materials and media. Groomingdale’s Salon and Paw-tique, LLC agrees not to sell or distribute these photos to any third party.
  • My dog will/is currently enrolled in the daycare program at Groomingdale’s Salon and Paw-tique, LLC and I have entered into a daycare program agreement to provide for my dog’s care.  As part of this agreement, I give my permission for Groomingdale’s Salon and Paw-tique, LLC; its officers, agents, owners, and employees to take my dog(s) outside of the facility on lead for additional training and socialization. I understand that the terms of the Daycare Agreement entered into with Groomingdale’s Salon and Paw-tique, LLC applies to my dog(s) both when he/she is inside the facility and also when he/she is taken outside for additional training, socialization or transportation.
  • Vaccinations and medical history: (attach copy) Bordetella vaccination 10 days prior to attending daycare is required for all dogs attending daycare, classes, and boarding. Bordetella vaccinations must be updated as recommended by your Veterinarian. Vaccinations and medical history can be obtained by GS and its officers, from your Veterinarian at any point in time.


Be aware that your dog being around other dogs gives them a higher chance of contracting parasites and illness. To protect your dog and others we highly recommend that you administer de-wormer and Bordetella every 6 months. We will do our best to keep the environment as clean as possible, but we do not know where our clients take their dogs when they are not with us.
